Athletic Forms

In order to participate in Athletics at BMHS please complete the following five forms.

Permission Form

This form includes the parent permission, insurance waiver, overnight stay, and code of conduct.

Physical Form

This form certifies that your child is physically able to participate in athletics.

Student Eligibility Form

This form is a competitor medical form from CHSAA and must be signed by each student and parent/guardian participating in athletics.

Transportation Form

This form releases Bishop Machebeuf from any liability while students are driving to and from athletic practices or competitions.

Concussion Awareness

This form states that you have been educated about concussions in sports.

Student Accident Insurance Program

Protect your child with student accident insurance. If you don’t have other insurance, this student accident insurance is vital. If you have other insurance, student accident insurance can help with deductibles and copays.
